Hospice Care

Hospice care is end-of-life care provided by health professionals and volunteers. They give medical, psychological and spiritual support. The goal of the care is to help people who are dying have peace, comfort and dignity. The caregivers try to control pain and other symptoms so a person can remain as alert and comfortable as possible. Hospice programs also provide services to support a patient’s family.

Usually, a hospice patient is expected to live 6 months or less. Hospice care can take place
•At home
•At a hospice center
•In a hospital
•In a skilled nursing facility

Most people want their last days to be in the comfort of their own home, surrounded by the warmth of their own family and home. Hospice Care enables the majority of people to accomplish this in a dignified and peaceful manner through a large network of Social Workers, Bereavement Specialists, Spiritual Support and Case Managers. However, the professionals in Hospice Care are not able to be there continually. At Constant Companions Home Care, San Diego and S. Riverside, we provide families with compassionate, empathetic and experienced hospice caregivers that can be there for as little as 4 hours a day to 24 hours a day. Every caregiver we hire has to go through a stringent application process see About Our Caregivers.  ONLY caregivers with verified hospice experience are placed with our hospice clients.

Over the past 9 years we have worked hand-in-hand with most San Diego and S. Riverside hospices. Our care coordinators and caregivers team up with the case managers of each hospice organization to assure that all direction and instruction are carefully followed to assure the highest quality of life for each client.
