Compassionate Home Care Services
Usually, by the time our office receives a call, family and friends have been very involved for an extended period of time, providing most of the daily care. The call comes because the family is in need of support and guidance.
We continually strive to provide the best home care experience possible. During our extensive intake process, we identify where the home care worker can provide support for family members, as well as the care recipient. Our goal is to match you with a caregiver that can best fulfill your needs and expectations using our proprietary Best-Fit Method

End-of-Life Hospice Care
What is Hospice Care?
Hospice care is end-of-life care provided by health professionals and volunteers. Hospice care workers give medical, psychological, and spiritual support. The goal of the care is to help people who are dying have peace, comfort, and dignity. The focus is on a person’s overall quality-of-life rather than attempting to cure. Hospice programs also provide services to support a person and their family through this end-of-life transition.
With Constant Companions San Diego and Riverside, families can count on compassionate, empathetic and experienced hospice caregivers. Hire a caregiver for as little as 4 hours a day to 24 hours a day. Every caregiver we hire has to go through a stringent application process. ONLY caregivers with verified hospice experience are placed with our hospice clients.
Over the years we have worked hand-in-hand with most San Diego and Riverside hospices. Our care coordinators and caregivers team up with the case managers of each hospice organization to assure that all direction and instruction are carefully followed to assure the highest quality of life for each client and maximum support for all family members.
A Hospice patient has a prognosis of 6 months or less. Hospice care can take place:
• At home
• At a hospice center
• In a hospital
• In a skilled nursing facility
Most people want to be in the comfort of their own home, surrounded by the warmth of their own family and friends. Hospice Care enables the majority of people to accomplish this in a dignified and peaceful manner through a large network of Social Workers, Bereavement Specialists, Spiritual Support and Case Managers. However, the professionals in Hospice Care are not able to be there continually. Constant Companions is able to fill that need with caring and compassionate caregivers 4-24 hours a day.
